Physical Therapist - Home Health - (PT - HH)

This Home Health Physical Therapist position is based in Lewistown, Pennsylvania, a welcoming central Pennsylvania community known for its small-town charm, reasonable cost of living, and strong sense of community. As a Home Health PT, you will provide one-on-one, skilled physical therapy services in patients’ homes throughout Lewistown and neighboring communities. You will perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions aimed at improving mobility, function, and safety in the home environment. Typical diagnoses include post-surgical orthopedic conditions, balance and gait disorders, neurologic conditions, cardiopulmonary impairments, and generalized deconditioning.
Your typical day will include traveling between patient homes, performing 5–7 visits per day depending on geography and patient needs. You will complete initial assessments, revisit treatments, and discharge assessments, ensuring that each plan of care reflects the patient’s goals and home situation. You will focus heavily on functional mobility training, transfer training, gait and balance work, strengthening, endurance training, home exercise programs, and fall prevention strategies. Home safety assessments and recommendations for assistive devices or adaptive equipment will be a regular part of your role.
You will collaborate closely with an interdisciplinary home health team that may include nurses, occupational therapists, speech therapists, social workers, and home health aides. Communication and coordination are central to ensuring patients receive cohesive, high-quality care. Documentation is completed via an electronic medical record system, and you will be expected to maintain timely, accurate visit notes and care plan updates.
Schedules typically include daytime hours with some flexibility to accommodate patient needs and drive times. Weekend or occasional on-call rotations may be required depending on census and team structure. While there is no fixed patient ratio in home health, your caseload will be managed to support safe driving distances and appropriate visit lengths, allowing time for meaningful patient interaction.
