Job Opportunity – Physical Therapist, Home Health – Portland, OR 97225
Start Date: September 7, 2026
Location: Portland, Oregon and surrounding areas
Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. Up to 6 patient visits per day with a standard productivity expectation of 30 points. No weekend or on-call requirements.

Requirements:- Active Oregon Physical Therapist license required
- Minimum 1 year of skilled home health experience required
- OASIS documentation experience required
- Basic Life Support (BLS) certification required
- EPIC experience preferred (EPIC Remote Client for Home Health and Rover app)
- Reliable personal transportation required
- COVID-19 primary vaccination series plus one booster required
- Two professional references covering 1 year within the last 3 years
