This role involves providing both direct and indirect patient care services in patients' homes, following physician-approved care plans. As part of an interdisciplinary team, the therapist plays a critical role in enhancing functional mobility and activities of daily living through specialized treatment programs.

Key Responsibilities of the Physical Therapist- Evaluate patient needs and develop individualized treatment programs
- Administer complex therapeutic exercises for improved functional mobility
- Instruct and motivate patients and caregivers on activities of daily living
- Provide clinical supervision to Physical Therapist Assistants and Home Health Aides
- Ensure appropriate use of adaptive equipment
- Maintain effective communication with the interdisciplinary care team
Requirements of the Physical Therapist- CO License or compact license accepted
- Experience with HCHB and Home Health
- OASIS experience
