Job Summary (List Format): Physical Therapist (SNF Setting)
- Conduct initial and ongoing assessments of residents mobility, balance, strength, gait, and functional abilities.
- Develop individualized, person-centered treatment plans to restore/improve physical function, reduce pain, and prevent disability.
- Deliver direct physical therapy interventions, including therapeutic exercises, gait and balance training, and use of modalities.
- Monitor residents progress, adjust care plans as needed, and document all therapy sessions and related information.
- Collaborate with interdisciplinary care teams, participate in meetings, and contribute to discharge planning.
- Educate residents, families, and caregivers on exercise programs and safety for promoting independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure standards.
- Ensure compliance with physician orders, facility policies, and all state and federal practice regulations.
- Support quality assurance and performance improvement initiatives within the rehab department.
- Work in a skilled nursing facility (SNF)/long-term care environment, requiring frequent standing, bending, lifting, and assisting residents.
