Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: 8 hours shifts Tuesday - Saturday Hours per Shift: 8 Experience: Minimum 2 years License: Required Certifications: Required Description: Our facility is looking for a Physical Therapist (PT) to support the treatment of illness and/or injury through the use of physical exercises and therapies. This role assists in the rehabilitation and restoration of body functions to promote maximum client wellness and independence. You will provide therapy programs for patients of all ages with developmental deficits, physical injury, or illness. The role may include other responsibilities and duties as assigned based on facility needs and requirements. Responsibilities: Coordinate PT services, providing assigned clients with direct therapeutic care. Teach rehabilitation skills to clients and/or caregivers that promote maximum independent functionality and safety. Provide preventive and rehabilitative PT procedures individualized to clients according to age-specific needs. Perform all duties within the scope of physical therapy practice guidelines and in accordance with productivity standards. Develop and revise plans of care based on initial assessments and continual reassessment of client needs. Monitor the application of treatments involving physical agents such as heat, water, and electricity, using appropriate equipment. Follow utilization guidelines and protocols. Formulate and implement appropriate teaching plans, and communicate significant clinical findings and changes. Document all skilled and medically necessary care thoroughly, timely, and accurately. Ensure plans of care and documentation comply with regulatory and insurance requirements. Maintain client and organizational confidentiality at all times. Participate in the Quality Management program and complete required in-service programs. Conduct supervisory visits and prepare updates for Home Health Aides as applicable. Supervise Physical Therapy Assistants in accordance with relevant laws, regulations, and agency policy. Participate in meetings and programs as required to meet facility goals and obligations. Deliver a high level of customer service to all internal and external customers. Demonstrate leadership competencies through communication, relationship building, and teamwork.
