Description:
This position follows a clinical interview process.
Candidates can expect outreach within two hours via phone and text where applicable.
Decisions are typically made the same day or by the next business day.
Applicants seeking more than 7 days off or more than 50% of holidays off are not eligible for the same processing timeline.
Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care.
Develops appropriate plans of care following all regulatory and clinical practice standards.
Utilizes standardized assessments and evidence-based practice to support clinical interventions.
Administers various procedures as part of the rehabilitation plan including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements.
Evaluates treatment outcomes for effectiveness.
Services are modified in relation to patient evaluation and referrals are made based on outcomes.
Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
Provides comprehensive discharge summaries as required.
Documents all regulatory and clinically required information in the electronic documentation system.
Maintains patient records according to company policy.
Accurately records treatment minutes for services provided.
Participates in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings.
Participates in facility and company required trainings, in-services, and conferences.
Able to obtain any necessary Durable Medical Equipment or supplies per regulatory and clinical practice requirements.
Supervises and supports physical therapist assistants, rehabilitation aides, and/or students.
Adheres to required practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
No weekends unless a holiday falls within M-F; hours can be made up on the weekend.
