Key Responsibilities of the Physical Therapist- Perform physical therapy evaluations and treatments per physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, medical history, and necessary precautions
- Develop and implement treatment plans with measurable goals
- Regularly reassess treatment plans and adjust as needed
- Communicate patient needs and progress with the treatment team and relevant parties
- Consult with medical providers regarding patient care and discharge
- Recommend and assist with adaptive equipment for patients
- Maintain documentation in compliance with regulations and standards
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development and continuing education
Requirements of the Physical Therapist- Valid DPT license
- Proficiency with HCHB electronic charting system
- Knowledge of disease processes and patient education
