Description

Key Responsibilities
  • Complete evaluations to ensure quality standards and address assessment areas requiring attention
  • Establish individualized treatment plans and provide therapy interventions using appropriate protocols and techniques
  • Develop individualized care and discharge plans and communicate them to appropriate parties
  • Complete comprehensive and accurate clinical documentation, including OASIS, within established deadlines
  • Supervise assigned therapy assistants as applicable
  • Serve as co-leader in an interdisciplinary approach to patient care
Required Qualifications
  • Active Physical Therapy licensure in the state of practice
  • Doctor of Physical Therapy (DPT) degree required if license was obtained on or after January 2016, or foreign equivalent
  • AHA BLS or CPR certification from an approved vendor required
  • Valid Driver's License; must be 21 years of age or older
  • Minimum 1 year of home health experience required
  • Proficiency in HCHB, OASIS, and EPIC required
Preferred Experience / Skills
  • At least two years of recent clinical experience in the field
  • Experience working in a critical access or rural healthcare environment
  • Familiarity with interdisciplinary care team models
