Key Responsibilities

• Provide physical therapy services to patients across an assigned multi-city territory in the Alameda County and East Bay area
• Conduct evaluations, re-evaluations, OASIS assessments (SoC, RoC, ReCertification, Discharge, Follow-up), routine visits, and discharge visits
• Maintain expected weekly productivity of at least 27 points per week to meet caseload requirements
• Travel within and, when necessary, outside the primary service area to meet weekly visit expectations
• Weekend coverage may be required based on caseload needs
• Willingness to see Covid-positive and Covid PUI patients is required

Required Qualifications

• Current California Physical Therapist (PT) License
• BLS Certification
• Valid Driver's License, current auto insurance, and current vehicle registration
• Up-to-date health documentation required: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual)
• Must pass pre-placement drug screen and physical

Preferred Experience / Skills

• Prior home health physical therapy experience
• Familiarity with OASIS documentation and home health visit workflows
• Comfort managing a high-volume, multi-city caseload independently
