Key Responsibilities
Evaluate and treat patients in accordance with individualized physical therapy plans of care
Document patient progress, treatment sessions, and clinical outcomes in a timely manner
Collaborate with interdisciplinary care team to support patient rehabilitation goals
Maintain a guaranteed 40-hour work week as outlined in contract terms
Must carry and present a valid state or federal issued photo identification at the facility at all times
Required Qualifications
Active Physical Therapist (PT) license in the state of New Hampshire
Graduate of an accredited Physical Therapy program
Valid state or federal issued photo identification required on-site at all times
Preferred Experience / Skills
Prior experience in a skilled nursing or long-term care setting
Experience with geriatric patient populations
Strong documentation and communication skills
