Description:
This position follows the clinical interview process.
Candidates can expect outreach within two hours via phone and text where applicable, with decisions typically made the same day or by the next business day.
Applicants who are looking for more than 7 days off or who seek more than 50% of holidays off are not eligible for the same processing timeline.
If a new graduate, the candidate will not go through the clinical interview process but will be presented to the facility for review.
Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care.
Develops an appropriate plan of care following all regulatory and clinical practice standards.
Utilizes standardized assessments and evidence-based practice to support clinical interventions.
Administers various procedures as part of the rehabilitation plan, including but not limited to manual techniques, ambulation, therapeutic exercises, modalities, and the use of supportive and assistive devices.
Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements.
Evaluates treatment outcomes for effectiveness.
Modifies services in relation to patient evaluation and makes referrals based on outcomes.
Provides consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
Effectively communicates with supervisors and other health team members about patient progress, barriers, and treatment plans.
Provides a comprehensive discharge summary of services provided per regulatory and clinical requirements.
Documents all regulatory and clinically required information in the electronic documentation system.
Maintains patient records according to company policy.
Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
Participates in required meetings, including patient care conferences and utilization review meetings.
Participates in facility and company required training, in-services, and conferences.
Able to obtain any necessary durable medical equipment or supplies per regulatory and clinical practice requirements.
Able to supervise and support physical therapist assistants, rehabilitation aides, and/or students.
Adheres to APTA and state-required practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
