Travel Physical Therapist (PT) - $2,340 per week in Chapel Hill, NC

The PT will evaluate patients, develop individualized treatment plans, and provide therapy focused on improving mobility, strength, balance, functional independence, and overall quality of life. Provide therapeutic exercises, functional mobility training, gait training, balance activities, and strengthening programs.

We are seeking a skilled and compassionate Physical Therapist (PT) to provide quality rehabilitation services in a Skilled Nursing Facility (SNF) setting. The PT will evaluate patients, develop individualized treatment plans, and provide therapy focused on improving mobility, strength, balance, functional independence, and overall quality of life.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, functional mobility training, gait training, balance activities, and strengthening programs.
• Treat patients with a variety of diagnoses and functional limitations commonly seen in the SNF setting.
• Assist patients with transfers, ambulation, positioning, and use of assistive devices.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients, families, and caregivers on exercises, safety, mobility, and home programs.
• Maintain accurate and timely documentation in accordance with facility and regulatory requirements.
• Communicate patient progress and recommendations with the interdisciplinary healthcare team.
• Participate in care planning, discharge planning, and coordination of services.
• Promote patient safety and maintain a supportive rehabilitation environment.

Requirements

• Graduate of an accredited Physical Therapy program.
• Current PT license in the applicable state.
• Previous SNF or post-acute rehabilitation experience preferred.
• Strong clinical, communication, and documentation skills.
• Ability to work independently and collaboratively with an interdisciplinary team.
