Travel Physical Therapist (PT) - $2,317 to $2,517 per week in Dallas, TX

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• At least 1 year home health experience
• Experience with HCHB
• Physical Therapist lisenced in the state of TX

Assignment Details

• Shift hours: 8:00 AM - 4:30 PM
• Shift days: Monday - Friday
• On-call requirement: 1 weekend per month
• Coverage area: Dallas service area
• Productivity expectation: 30 points weekly
• Primary responsibilities include OASIS and subsequent visits
