Key Responsibilities
Conduct physical therapy evaluations and treatments during in-home patient visits
Travel between patient homes to deliver care, managing a dynamic and changing daily schedule
Complete and maintain OASIS assessments in accordance with home health requirements
Provide care in both home health and hospice settings as needed
Document patient progress and clinical notes using the HealthWyse EMR system
Required Qualifications
Active Physical Therapist (PT) license in the state of Vermont
OASIS experience required
Home health experience required
Hospice experience required
Must have experience traveling between patient locations for home visits
Must be able to manage and adapt to frequent schedule changes
Preferred Experience / Skills
Familiarity with the HealthWyse EMR platform
Experience working in outpatient or community-based care settings
