Job Title: Physical Therapist Profession: PT (Physical Therapist) Specialty: PT (Physical Therapy) Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8 hours Experience: Minimum of 1 year License: Required Certifications: BLS (AHA) Must-Have: Ability to perform Oasis Assessments and Start of Care cases Description: Serves primarily rural areas with mid-sized towns. Shift typically runs from 8 AM to 5 PM, Monday through Friday. Facility uses a point system equivalent to 4-6 patient visits daily. A tablet is utilized to track time and visits. Business casual attire or hunter green scrubs with a white or black top are required. Common cases include elderly falls and fractures. The facility may cancel up to 4 shifts per 13-week assignment and 3 shifts for assignments that are shorter than 13 weeks. Charting system used is Homecare Homebase (HCHB). Trauma level is unspecified. Care setting is Home Health. Division is Allied Health. Shift notes reflect Monday through Friday, from 8 AM to 5 PM.
