License: Possession of a current Client License in Physical Therapy.
Certifications: BLS (AHA ONLY)

Description:
Provides case management and direct patient care within a team environment.
Administers comprehensive physical therapy modalities in the home setting.
Assists the physician in evaluating levels of function and helps develop the plan of care, revising it as necessary.
Prepares clinical and progress notes.
Advises and consults with families and other personnel.
Participates in in-service programs.
Maintains effective working relationships with patients, families, and staff.
Utilizes resources, supplies, and time efficiently to contain costs and increase the value of all services.
Displays positive, supportive, and respectful communication to all external sources.
Strives for continuous improvement in all aspects of the Physical Therapist function and care delivery process.
Develops and maintains a customer-oriented environment responsive to the needs of all customers.
Develops and revises treatment and discharge plans in conjunction with patients and families.

Initiates appropriate rehabilitative programs.
Provides supervision of physical therapy assistants to ensure patient health, safety, and compliance with the plan of care.
Educates patients, families, and team members regarding disease processes, self-care techniques, and preventive strategies.
Implements home therapy programs, including but not limited to: gait training, transfers, activities of daily living, home modifications, and use of equipment.
Periodically evaluates patient progress and makes necessary adjustments to physical therapy programs.
Instructs para-professional staff in body mechanics, use of equipment, bed mobility, transfers, gait patterns, and home exercise programs.
Coordinates and supervises the quality of patient care services provided by home health staff.
Interacts with patients and others in a manner conducive to continued positive relationships.
Ensures problem resolution at the point of contact.
Participates in patient conferences with other providers for the purpose of discussing patient care and ensuring a comprehensive coordinated treatment plan.
Assists in the orientation of new rehabilitation personnel.
Completes all necessary clinical records for compliance with regulations.
Refers to appropriate community resources.
Maintains current knowledge of available community services.
Ensures patient needs are met and responds to customer needs effectively.
Ensures practices and behaviors meet agency performance competencies.
Participates in the attainment of operational and financial objectives.
Monitors results and takes corrective action on quality indicators.
