Job Title: Physical Therapist Profession: Physical Therapist Specialty: Occupational Therapist Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8 hours Experience: Minimum of one year experience in Long Term Care/Home Care setting as Physical Therapy, recent Home Care experience (within the last year) License: Possession of a current Client License in Physical Therapy Certifications: BLS (AHA only) Must-Have: Oasis experience Description: Provides case management and direct patient care within a team environment. Administers comprehensive physical therapy modalities in the home setting. Assists the physician in evaluating the level of function. Helps develop the plan of care and revises it as necessary. Prepares clinical and progress notes. Advises and consults with the family and other personnel. Participates in in-service programs. Promotes effective working relationships and works effectively as part of the team. Utilizes resources, supplies, and time efficiently to contain costs. Communicates positively and respectfully with all external sources, patients, families, and staff. Strives for continuous improvement in all aspects of the Physical Therapist function. Develops and maintains a customer-oriented environment responsive to the needs of all external/internal customers. Develops and revises plans of treatment and discharge in conjunction with patients, families, and physicians. Provides an environment that promotes respect for patients, their privacy, and confidentiality. Implements appropriate rehabilitative programs. Provides supervision of assistants to ensure patient health, safety, and compliance with the plan of care. Consults with and educates patients and families regarding disease processes and self-care techniques. Implements home therapy programs including gait training, transfers, and activities of daily living. Evaluates patient progress and adjusts the physical therapy program as necessary. Instructs para-professional staff in body mechanics and the use of equipment. Directs and evaluates the quality of patient care services provided. Interacts with patients and physicians to ensure continued positive relationships. Assures collaborative customer-focused planning and case management. Participates in regularly scheduled patient conferences for discussing care and concerns. Assists in the orientation of new rehabilitation personnel. Completes all appropriate clinical records for compliance with legislation. Refers to appropriate community agencies and maintains current knowledge of available services. Displays flexibility and enthusiasm while working with customers and others. Makes constructive recommendations regarding quality of care and operations. Ensures that patients' needs are met through the care delivery process. Participates in the attainment of operational and financial objectives. Monitors quality indicators and takes corrective action as needed. Contributes to an environment of professional growth and mutual respect. Participates in education and continuing development opportunities. Demonstrates commitment to professional growth and competency.
