Key Responsibilities
Evaluate and treat residents within a continuing care retirement community setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward established goals
Collaborate with interdisciplinary care teams to support resident outcomes
Participate in weekend coverage as needed
Required Qualifications
Active Physical Therapist (PT) license in the state of California
Graduate of an accredited Physical Therapy program
Ability to work a schedule that includes some weekend shifts
Preferred Experience / Skills
Prior experience in a CCRC, skilled nursing, or long-term care environment
Experience working with geriatric populations
