Seven Healthcare are seeking a dedicated Physical Therapist for a contract-to-hire position based in Ronceverte, WV. This role involves providing comprehensive homecare services. As a Physical Therapist, you will perform evaluations and treatments based on physician orders, with the support of a dynamic team and the flexibility to operate within a travel assignment framework.

Key Responsibilities of the Physical Therapist- Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
- Complete a comprehensive evaluation using appropriate tools and procedures
- Review patient records for diagnosis, past medical history, precautions, and contraindications
- Establish measurable goals and develop a treatment plan for patient independence and function
- Regularly reassess the treatment plan, patient progress, and need for modifications
- Communicate patient needs and progress to the team, physician, and relevant parties
- Consult with medical providers on patient evaluation, treatment, and discharge
- Recommend and assist in securing adaptive equipment for patient function
- Maintain and submit documentation in required formats and timelines
- Adhere to professional standards, company policies, and state/federal laws
- Supervise Physical Therapist Assistants and Aides in patient care activities
- Participate in staff development programs, meetings, and continuing education
Requirements of the Physical Therapist- WV PT license or Compact license accepted
- Experience in Homecare, OASIS, HCHB, Exercise programming, and Rehab
