Job Title: Physical Therapist

Location: Bayonne, NJ

Pay: $2,156 weekly

Shifts: Monday thru Friday 8am-5pm and one weekend a month

Seven Healthcare are seeking a skilled Physical Therapist for a contract-to-hire position in Bayonne, NJ. This travel role is focused on providing physical therapy evaluation and treatment based on physician orders and referrals in an onsite setting. As a key member of the treatment team, the Physical Therapist will work towards enhancing patients' independence and function.

Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluations and treatments based on physician orders/referrals
  • Complete comprehensive evaluations using appropriate tools and procedures
  • Review patient records for diagnosis, history, precautions, and contraindications
  • Develop personalized treatment plans to achieve patient independence/function
  • Re-assess treatment effectiveness and modify plans/goals as needed
  • Communicate patient progress to treatment team, physicians, and family
  • Recommend adaptive equipment and incorporate it into rehab programs
  • Maintain treatment and billing documentation as per regulations
  • Adhere to professional standards and legal requirements
  • Supervise Physical Therapist Assistants and Aides
  • Participate in staff development programs and maintain clinical skills

Requirements of the Physical Therapist
  • Physical Therapy License in NJ
  • Ability to achieve at least 30 benchmark points per week
  • Timely documentation within 24 hours
  • Preferred skills in Home care home base (HCHB), Point care, Point click care, Microsoft Teams
