Seven Healthcare is seeking an experienced Physical Therapist for a contract position in Ashland, KY. Key Responsibilities of the Physical Therapist- Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
- Complete comprehensive evaluations using appropriate assessment tools and procedures
- Review patient records for current diagnosis, past medical history, precautions, and contraindications
- Establish measurable goals and develop treatment plans to enhance patient independence and function
- Regularly re-assess treatment plans and patient progress, modifying goals or treatment as needed
- Communicate patient needs and progress to the treatment team, physicians, and family members
- Consult with medical providers regarding patient evaluation, treatment, and discharge plans
- Assist in securing special adaptive equipment for patients to improve function
- Maintain and submit documentation according to company, facility, state, and federal regulations
- Adhere to the Company's Code of Business Conduct, policies, and legal standards
- Supervise Physical Therapist Assistants and Aides in direct patient care and related activities
- Participate in internal staff development programs and maintain clinical practice skills
Requirements of the Physical Therapist- KY Physical Therapist Licensure, Compact Licensure Accepted
- 1 year of home health physical therapy experience
- Previous OASIS experience
- Experience with Home Care Home Base
