Physical Therapists at VNH screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.

The PT will:

  • Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
  • Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.
  • Communicate with all members of the health care team.
  • Document evaluations, treatment goals, and objectives.
  • Teach patients and family members the rationale for treatments and procedures.

Minimum Qualifications:

  • BS in Physical Therapy
  • Must be a licensed Physical Therapist in VT and NH, or hold an unencumbered compact license.
  • One year of experience as a PT in a home healthcare setting
