Description

Key Responsibilities
  • Provide physical therapy services to patients in their homes across an assigned multi-county territory, traveling up to 75 miles from the home clinic
  • Manage a caseload of approximately 5-10 patients per day, including Start of Care and Follow-Up visit types
  • Meet weekly productivity expectations of 30 points per week, where each visit type and activity carries an assigned point value
  • Complete all patient documentation within 24 hours of each appointment using HomeCare HomeBase EMR and OASIS
  • Attend periodic in-office meetings at the assigned care center
  • Participate in Saturday Start of Care rotations on a rotating monthly basis as needed
Required Qualifications
  • Active, unrestricted Physical Therapist license in hand at time of submission (no pending licenses accepted)
  • CPR/BLS certification required
  • Minimum of 1 full year of recent adult home health experience - first-time home health candidates will not be considered
  • Proficiency in HomeCare HomeBase EMR within the last 6 months (or willingness to complete a retraining course during compliance)
  • Must have a reliable personal vehicle, valid driver's license, and agency-required auto liability insurance
  • Must be willing and able to travel up to 75 miles from the assigned care center and accept all patients on assigned schedule regardless of distance from previous appointment
  • Local candidates are welcome and accepted at the same rate
Preferred Experience / Skills
  • Experience with OASIS documentation used within the last 6 months
  • Familiarity with Start of Care assessments and follow-up visit workflows in a home health setting
  • Comfort managing a geographically dispersed patient caseload with variable drive times between appointments
