Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting
• Evaluate, develop, and implement individualized treatment plans to support patient recovery and functional goals
• Travel to patient homes within the designated service area (approximately 25% travel required)
• Document patient progress and maintain accurate clinical records in accordance with regulatory and organizational standards
• Collaborate with interdisciplinary care teams to coordinate patient care

Required Qualifications

• Active California Physical Therapist (PT) license required
• Basic Life Support (BLS) certification required
• Valid driver's license and reliable transportation for patient home visits

Preferred Experience / Skills

• Prior experience in a home health or community-based therapy setting
• Strong time management and independent clinical decision-making skills
