Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Complete required clinical documentation in a timely and accurate manner
• Develop and implement individualized treatment plans based on patient evaluations
• Collaborate with interdisciplinary team members to support patient progress and outcomes
• Maintain compliance with credentialing and facility-specific requirements prior to and during assignment

Required Qualifications

• Active Physical Therapist license required; compact license accepted at start but permanent Oregon licensure must be obtained promptly
• Experience in an outpatient physical therapy setting
• Must meet contract QA credentialing requirements prior to assignment start

Preferred Experience / Skills

• Prior experience working in a high-volume outpatient therapy environment
• Strong documentation skills and familiarity with electronic medical records
