Key Responsibilities- Provide direct physical therapy services to patients in an outpatient setting
- Complete required documentation in accordance with facility and credentialing standards prior to assignment start
- Evaluate, plan, and implement individualized treatment programs based on patient needs
- Maintain accurate and timely clinical documentation throughout the course of care
- Collaborate with interdisciplinary team members to support optimal patient outcomes
Required Qualifications- Active Oregon Physical Therapist license required - permanent state license required; compact license alone is not sufficient
- Experience in an outpatient physical therapy setting
- Must meet contract credentialing and quality assurance requirements prior to start
- 40 guaranteed hours per week; 32 guaranteed hours during holiday weeks
Preferred Experience / Skills- Prior experience working in a high-volume outpatient therapy environment
- Strong documentation skills and familiarity with outpatient EMR systems
