Key Responsibilities
Evaluate and treat patients in an outpatient physical therapy setting
Develop and implement individualized treatment plans based on patient needs and goals
Complete required clinical documentation accurately and in a timely manner
Collaborate with the care team to ensure continuity and quality of patient care
Maintain compliance with facility credentialing and quality assurance requirements
Required Qualifications
Active Physical Therapist license in the state of Oregon (permanent license required)
Experience in an outpatient physical therapy setting
Ability to meet contract credentialing and quality assurance requirements prior to start
Preferred Experience / Skills
Prior experience working in a high-volume outpatient orthopedic or sports rehabilitation environment
Strong documentation skills and familiarity with electronic medical records
