Key Responsibilities

• Evaluate and treat patients in an outpatient physical therapy setting
• Develop and implement individualized treatment plans based on patient needs and clinical findings
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
• Collaborate with interdisciplinary team members to support patient progress and care coordination
• Adhere to all credentialing, compliance, and facility-specific requirements prior to and during the assignment

Required Qualifications

• Active Physical Therapist license required; compact license accepted at start but permanent Oregon state licensure must be obtained promptly
• Experience in an outpatient physical therapy setting required
• Must meet contract credentialing and quality assurance requirements prior to assignment start
• Candidates who have worked at this facility in a permanent or PRN capacity within the last year are not eligible for this assignment

Preferred Experience / Skills

• Prior travel therapy experience in an outpatient setting
• Strong documentation skills and ability to adapt to facility-specific systems and workflows
