- Monday - Friday, 8 am - 4:30 pm, 8-hour shifts
- 40-hour guarantee per week
- Rotating weekends and holiday coverage required if needed
- Home health setting providing physical therapy services
- OASIS proficiency required -- non-negotiable
- 2+ years acute nursing care and home care experience required
- Valid driver license and personal vehicle required
- Must work independently with minimal supervision

Per week (based on a 40-hour week). Includes estimated taxable wages of $21.23 - $50.33 per hour and estimated tax-free reimbursements for meals, incidentals, and housing of $900 - $1,400 per week based on GSA guidelines (subject to eligibility, location, and seasonal adjustment).
