Key Responsibilities
Evaluate and treat patients in an outpatient physical therapy setting
Develop and implement individualized treatment plans based on patient needs and clinical findings
Document patient progress and maintain accurate clinical records in accordance with facility requirements
Collaborate with interdisciplinary team members to coordinate patient care
Maintain compliance with credentialing and facility-specific documentation requirements prior to and during assignment
Required Qualifications
Active Physical Therapist license required
Experience in an outpatient physical therapy setting
Must meet facility credentialing requirements, including completion of contract QA documentation
Preferred Experience / Skills
Prior experience working in a high-volume outpatient clinic
Strong documentation skills and ability to adapt to facility-specific systems
