Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Complete required clinical documentation in a timely and accurate manner
• Develop and implement individualized treatment plans based on patient evaluation and goals
• Collaborate with interdisciplinary team members to support patient progress and outcomes

Required Qualifications

• Active Physical Therapist license in the state of Oregon (permanent license required)
• Experience in an outpatient physical therapy setting
• Ability to meet facility credentialing requirements prior to assignment start
• Expected 40 hours per week; expected 32 hours per week during holiday weeks

Preferred Experience / Skills

• Prior experience working in a high-volume outpatient therapy environment
• Strong documentation and clinical communication skills
