Position: PT Home Health Home Office: 909 South 336th Street, Suite 100, Federal Way WA 98003 Reports to: Mara Jacobo Timecard Approver: Julian Whitford or Kristina Kizer Invoicing Contact: Julian Whiford and Kristina Kizer Reason for opening: exiting of other PTs Who will conduct Interviews: Julian Whitford and Mara Jacobo Position Details Experience needed: Home Health and HCHB Is there anything additional you think is important for us to screen for? no Number needed: 2 (one King and one for Pierce) Territory/coverage area: King Co and another one for Pierce co Schedule: Business Hours, full time Weekend Rotation: No On-Call: No EMR: HCHB What type of documentation equipment will be issued? (laptop, tablet, etc.): Phone will be issued PT - Points expectation weekly/daily: 25 points/units a week, about 5 points/units a day PT - Oversees work of PTA?
