Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Michigan Physical Therapist license required to start the assignment.
  • BLS (Basic Life Support) certification required from AHA or ARC.
  • Home Health experience required. Candidate must be certified or have demonstrated competency working in home health settings.
  • Experience with EPIC electronic health record preferred.
  • Ability to complete required documentation and travel to patient residences within assigned service area.
  • Preferred: prior travel or agency experience and experience managing caseloads in community or home-based care.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Monday - Friday, 08:00 - 17:00.
  • Holiday rotation: no weekends or holidays.
  • Start: 04/06/2026. Assignment length: 12 week.
