Contract | Days | Monday-Friday, 8:00 AM-4:30 PM, with one weekend day every four weeks Key Responsibilities
Evaluate and treat patients with physical, functional, or mobility-related conditions
Develop and implement individualized treatment plans based on patient needs and goals
Document patient progress and maintain accurate records using Epic EMR
Collaborate with interdisciplinary care teams to coordinate patient care
Educate patients and families on therapeutic exercises and home programs
Required Qualifications
Active Physical Therapist (PT) license in the state of Georgia or eligibility to obtain
All required certifications must be in-hand or candidate must indicate willingness to obtain
Epic EMR experience required
Valid Driver's License required
Preferred Experience / Skills
Prior experience in an acute care or hospital-based setting
Strong documentation and time-management skills
