Job Summary
Physical Therapist, Outpatient San Fernando. The Physical Therapist evaluates assigned patients and develops, implements, and modifies individualized treatment plans to support functional recovery and return to community activities. The role provides patient education, coordinates discipline-specific therapy services, and maintains accurate clinical documentation in an outpatient setting.
Job Details
- Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; current CPR Certification; active and applicable license for the state where work will be performed.
- Schedule: Day shift, 8 hours per day, Monday through Friday, 40 hours per week.
- Responsibilities: Complete comprehensive patient evaluations; use clinical reasoning to establish treatment plans and goals; provide care under valid physician orders; document evaluations, treatment goals, functional quality indicators, medical necessity, and charges accurately and timely; educate patients, families, students, and coworkers; participate in student development and supervision as assigned; maintain professional licensure and required continuing education; follow applicable state practice acts, regulatory requirements, safety procedures, and professional codes of conduct; contribute to quality and performance improvement initiatives.
- Additional Information: The role includes delegation and direction of discipline-specific therapy services, development of functional treatment programs, individualized professional development, participation in continuing education and professional activities, and adherence to patient confidentiality and safety standards.
