Job Summary:
Physical Therapist Outpatient (PT OP) in an outpatient setting. This position involves evaluating patients and developing individualized treatment plans to facilitate functional recovery and safe return to community activities. The role requires providing comprehensive, patient-centered care within a team-oriented environment.
Job Details:
- Certifications, Education, and Experience Requirements: Degree in Physical Therapy from an Accredited School; 6 months to 1 year of outpatient clinical experience; CPR Certification; 6 months of related outpatient experience is highly desired; minimum of 1 year of experience preferred.
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week, starting from 10/19/2026 to 01/18/2027.
- Completes comprehensive patient evaluations, utilizing clinical reasoning to develop and adjust treatment plans aimed at patient discharge within an established length of stay.
- Documents medical records clearly, accurately, and timely, in compliance with applicable state, federal, and organizational guidelines.
- Ensures all treatments are provided under signed, valid physician orders and insurance authorizations when necessary.
- Participates in student programs by providing a positive learning environment, supervision, and evaluations.
- Actively pursues ongoing professional development through continuing education, self-study, and involvement in professional organizations.
- Adheres to all applicable State Practice Acts, maintains licensure in good standing, and completes mandated continuing education.
- Contributes to performance improvement initiatives and provides feedback to enhance service quality.
- Demonstrates commitment to safety, including safe patient handling, infectious disease prevention, and slip/trip prevention.
- Follows all organizational policies regarding appearance, attendance, punctuality, and safety.
