    Description

    Location: Kingwood, WV
    Salary: Depends on Experience
    Description:
    Travel Physical Therapist – Home Health/Hospice
    Weekly Hour Guarantee: 40 hours per week
    Coverage Area: Kingwood, WV and the surrounding two counties
    Shift Schedule: Monday–Friday, 8:00 AM – 4:30 PM
    Rate: Negotiable and Dependent on Experience

    Top Responsibilities
    Provide skilled physical therapy services to patients in their homes
    Perform evaluations, develop individualized plans of care, and implement treatment interventions
    Monitor patient progress and adjust treatment plans as necessary
    Complete accurate and timely documentation in the electronic medical record
    Communicate effectively with physicians, care team members, patients, and families
    Ensure compliance with all home health and hospice regulations and agency policies

    Required Qualifications & Experience
    Active West Virginia Physical Therapist (PT) license
    Prior home health experience is required
