We’re seeking an experienced Travel Physical Therapist for a Home Health assignment in Sonora, California. This role is ideal for PTs who are comfortable working independently and managing a home health caseload.

Assignment Details

• Location: Sonora, CA

• Setting: Home Health

• Duration: 13 Weeks (extension possible)

• Schedule: Full-time (based on patient census)

Requirements

• Minimum 2 years of Physical Therapy experience – Preferred

• Home Health experience – Required

• HCHB (Homecare Homebase) experience – Required

• OASIS documentation experience – Required

• Active California PT License – Required

• BLS/CPR Certification – Required

• Valid Driver’s License – Required

Responsibilities

• Provide in-home physical therapy evaluations and treatments

• Complete OASIS assessments and documentation accurately

• Develop and implement individualized care plans

• Educate patients and caregivers on therapy plans and safety

• Collaborate with interdisciplinary home health teams

• Maintain compliance with state and federal home health regulations
