We are seeking an experienced Physical Therapist (PT) for an ASAP Home Health opportunity serving the West Valley of Phoenix, Arizona.
This is an excellent opportunity for a confident Home Health therapist who enjoys working independently, building relationships with patients and families, and helping patients improve their mobility and independence in the comfort of their homes.
Complete in-home Physical Therapy evaluations and reassessments.
Develop individualized plans of care based on each patient's functional needs and goals.
Provide therapeutic exercise, gait training, balance, mobility, and functional training.
Educate patients and caregivers on home exercise programs, safety, and fall prevention.
Coordinate care with physicians, nursing, therapy, and other members of the interdisciplinary team.
Complete accurate and timely clinical documentation.
Manage an assigned Home Health territory and patient caseload efficiently.
Graduate of an accredited Physical Therapy program.
Active Arizona Physical Therapist license in good standing.
Previous Home Health experience required.
Current BLS/CPR certification.
Valid driver's license and reliable transportation.
Strong time-management, communication, documentation, and independent clinical decision-making skills.
$2,359–$2,622 per week
