Description

American Traveler is seeking a Home Health Physical Therapist with CA PT licensure and Epic/OASIS experience to serve the South Marin County area.

Details

  • Home Health setting covering South Marin County towns including Sausalito, Tiburon, Belvedere, and Mill Valley
  • Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
  • Expected caseload of 5 visits/points per day
  • Uses Epic and OASIS for documentation

Requirements

  • Current CA PT license required if applicable
  • BLS certification required
  • Valid driver's license and auto insurance required
  • Minimum 1 year of experience required
  • Epic experience and OASIS experience required
  • Prior travel experience required
  • Strong critical thinking, independent judgment, and complex problem-solving skills required
  • Effective verbal and written communication and interpersonal skills required

Additional Information

  • Responsibilities include patient assessment, treatment planning, and delivery of home-based physical therapy care
  • Treats a broad range of conditions including orthopedic, neurologic, cardiopulmonary, and geriatric diagnoses
  • Collaborates with interdisciplinary team members and key stakeholders on patient care solutions
  • Provides patient and family education on home exercise programs, safety, and injury prevention
  • Maximum RTO of 7 days per contract, with only 1 holiday request approved per contract
  • Returning employees must have a 1-year separation before being eligible to return as a traveler
  • Local candidates accepted at local rate within a 50-mile radius
  • Lab work must be completed within 3-5 days of contract signing
