Job Description

  • Provide physical therapy services to patients in their homes.
  • Evaluate patients and develop individualized treatment plans.
  • Perform therapeutic exercises and mobility training to improve strength, balance, and independence.
  • Monitor patient progress and update treatment plans as needed.
  • Document patient care and coordinate with the healthcare team.
  • Weekend rotation every 4–6 weeks

Requirements

  • Home Health experience required
  • Active Michigan PT License required
  • BLS (AHA) required
