Job Summary
Physical Therapist (PT) Home Health Location: Not Specified. This role involves providing specialized physical therapy services to geriatric patients within a home health setting. The focus is on delivering clinical care in patients residences, ensuring therapy goals are met to improve mobility and quality of life.
Job Details
- Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
- Required Qualifications: Valid license in hand, minimum of 1 year relevant experience, current BLS certification, and proficiency in HCHB (Homecare Homebase).
- Preferred Qualifications: OASIS experience REQUIRED for staff completing Start of Care (SOC) assessments, recent home health experience.
- Responsibilities: Deliver physical therapy treatments to geriatric patients, complete all charting by 9 am the following day, and maintain timely communication.
- Additional Information: Flexibility with territory, and on-call rotation may be required. All charting must be completed by 9 am the following morning.
- Submission Requirements: Active drivers license, personal transportation, RTO (Requested Time Off) submitted at application, and Monday orientation attendance.
