Physical Therapist Home Health (PT HH) Located in Northern California, this position involves developing and implementing treatment programs aimed at restoring and enhancing patients' physical, social, and mental functions. The role functions as a co-leader within an interdisciplinary team to deliver patient-centered care in a community-focused healthcare setting.
Job Details
- Position Description: Plans and conducts segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.
- Required Qualifications: Doctor of Physical Therapy (DPT) degree (obtained license on or after January 2016) or foreign equivalent; physical therapy licensure in all applicable states of practice; Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification from approved vendor per AH policy; valid Drivers License and must be at least 21 years of age or older; AHA-BLS or AHA-CPR certification; familiarity with HCHB and OASIS; one year of home health experience; EPIC experience.
- Schedule: Days, 8 hours per shift, 40 hours per week, Monday through Friday, no weekends.
- Additional Information: Completes evaluations to ensure quality standards, uses appropriate clinical techniques and tools, establishes individualized treatment plans, provides therapy interventions, and ensures communication of discharge plans. Completes comprehensive clinical documentation within established deadlines. Supervises assigned assistants and performs other duties as assigned. All associates are required to receive all required vaccinations as a condition of employment and annually thereafter.
