Contract | Days | Monday–Friday, 8:00am–5:00pm, with occasional evening/weekend coverage
Key Responsibilities
- Evaluate patients and establish individualized plans of care within the home environment; coordinate care with other disciplines and obtain orders for additional services as appropriate
- Provide direct patient care in accordance with the established care plan and physician orders, applying evidence-based physical therapy practice guidelines, Medicare regulations, and Conditions of Participation
- Complete and master scoring of CMS-required OASIS assessments; collaborate with the Quality team on necessary adjustments
- Perform point-of-care documentation, recording all skilled services, adverse events, and relevant communications in the medical record
- Oversee services provided by Physical Therapist Assistants and conduct supervisory visits within required timeframes; assume supervision of CNAs when RN involvement in the care plan has ended
- Participate in multidisciplinary conferences, Quality Assurance Performance Improvement initiatives, and agency-provided educational opportunities
Required Qualifications
- Bachelor of Science degree or higher from an accredited Physical Therapy program
- Current Physical Therapist license to practice in the State of Montana
- Valid driver's license, reliable personal transportation, and appropriate auto insurance (required for travel to patients' homes within the service area)
- Understanding of, or willingness to acquire knowledge of, insurance requirements in Home Health
- Willingness to perform interventions permitted under the physical therapy practice act, which may include wound care
Preferred Experience / Skills
- Minimum of two years of prior physical therapy experience
- Familiarity with OASIS documentation and home health Medicare regulations
- Experience working in a home health or community-based care setting
