Job Summary
Physical Therapist Home Health Aspirus at Home Antigo. This position involves evaluating, treating, and planning patient care in accordance with Physical Therapy Standards of practice for adult and geriatric patients. The role requires working primarily within the home health setting, ensuring regulatory and reimbursement documentation, and maintaining productivity requirements.
Job Details
- Certifications: AHA Basic Life Support (BLS), Licensure required for the State of Wisconsin, NPI required (application process must be initiated if not already obtained).
- Education Requirements: Bachelor's Degree in Physical Therapy.
- Years of Experience: Minimum of 2 years of home health experience with training working with the adult/geriatric population presenting a variety of orthopedic, neurological, and functional mobility diagnoses.
- Schedule: Monday through Friday, 8:00 am to 5:30 pm, with a guaranteed 40 hours per week and flexibility up to 40 hours, including occasional travel to other branches as needed.
- Additional Information: Candidates should have knowledge of Physical Therapy standards and practices, hold a valid licensure for Wisconsin, and demonstrate resilience, social engagement, and the ability to manage change effectively. During flu season, unvaccinated travelers are required to wear masks at all times, in accordance with organizational policy.
