Job Summary:
Physical Therapist (PT) specializing in Home Health (HH) services in the Boston area. This role involves delivering comprehensive physical therapy care to patients in their residences, addressing impairments, functional limitations, and disabilities resulting from injury, disease, or other causes. The position requires collaboration with patients, families, physicians, and other health professionals to coordinate individualized care plans. The therapist will utilize electronic documentation and interdisciplinary communication in accordance with agency policies and standards of practice.
Job Details:
- Location: Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, West End
- Schedule: Days; 8 hours per shift; minimum of 40 hours per week
- Required Qualifications: Valid physical therapist (PT) license in Massachusetts; 1 year of home care experience; 1 year of case management experience; ability to travel to cases as assigned; commitment to working in a certified home health agency setting; proficiency with electronic documentation and interdisciplinary communication
- Preferred Qualifications: Experience in hospice, private care, or working solely as a case manager is not considered relevant for this position unless within a certified home health agency
- Additional Information: The candidate's start date is contingent upon credentialing clearance; if not cleared by the posted start date, a rescheduled start date will be necessary. Applicants must submit a resume, references, skills checklist, and submission coversheet for consideration.
