Description

American Traveler is seeking a Home Health Physical Therapist with 1+ year of skilled home health experience to serve patients in the West Portland, OR area.

Details

  • Home health setting covering West Portland, OR and surrounding areas
  • Day shift schedule, 8-hour shifts (5x8), 8:00 AM to 4:30 PM
  • 1:1 patient ratio with up to 6 patient visits per day; 30 productivity points standard
  • No weekend or on-call requirements
  • No floating between units required
  • Charting completed via Home Care Home Base (HCHB), Remote Client, and Rover phone app, with Oasis documentation
  • Occasional daily travel radius up to 60 miles (60-90 minutes total drive time)

Requirements

  • License required if applicable
  • BLS certification required
  • Minimum 1 year of skilled home health experience required
  • Oasis experience required for staff completing Start of Care (SOC)
  • Proficiency in HCHB (Homecare Homebase) required
  • Must have personal transportation and an active driver's license

Additional Information

  • Responsibilities include home-based physical therapy evaluation and treatment with minimized drive time through efficient client grouping
  • Primary series COVID vaccination plus one booster required
  • Business casual attire required; scrubs allowed but not mandatory, no jeans
  • Monday orientation includes completion of billable training modules
  • Hospice/Home Health checklist completion is mandatory
  • Travel pairs accepted, though each traveler is assigned their own territory and must have personal transportation
  • Local candidates accepted within a 50-75 mile radius at the same rate
  • First-time travelers are not accepted for this role
  • Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
  • Requested time off must be submitted at the time of application
