Description

American Traveler is seeking a Home Health Physical Therapist with 1+ years of experience to serve patients across the Dayton, Ohio area.

Details

  • Full-time home health position covering Dayton, Huber Heights, Fairborn, and Yellow Springs territories
  • Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
  • HomeCare HomeBase charting experience preferred but not required

Requirements

  • Minimum 1 year of experience required
  • BLS certification required
  • Active OH PT license required prior to start date; cannot start with a pending license
  • License required if applicable
  • Experience with acute care setting preferred
  • Skilled in assessment, evaluation, discharge planning, and development of long and short term goals
  • Proficient with OASIS and plan of care documentation
  • Experience with assistive devices including crutches, gait belts, rolling walkers, standard walkers, and wheelchairs
  • Knowledge of home exercise programs, patient and family education, and safety and injury prevention
  • Familiarity with Medicaid, Medicare A, and Medicare B billing

Additional Information

  • Responsibilities include conducting patient assessments, identifying environmental barriers, and performing pain assessments
  • Focus on patient and family education for safety and injury prevention in the home setting
  • Open to first-time travelers
