Job Summary
Physical Therapist Home Health (PT HH) Various Locations. This position involves providing outpatient therapy services to geriatric patients in a home health setting. The role requires recent experience with Hospice and Home Health care, proficiency in HCHB (Homecare Homebase), and adherence to regulatory standards to support patient recovery and mobility.
Job Details
- Specialty: Physical Therapist Home Health
- Schedule: Monday through Friday; 8-hour shift; minimum of 40 hours per week
- Certifications: BLS (Basic Life Support) required
- Experience: Minimum of 1 year of relevant experience, including OASIS experience for staff completing Start of Care (SOC) in Home Health, proficiency in HCHB (Homecare Homebase), and recent clinical practice in a home health or hospice setting
- License: Active in-hand license required, with submission upon application
- Additional Requirements: All travelers must have an active drivers license and personal transportation; timely communication and documentation are mandatory
- Clinical Responsibilities: Provide skilled therapy to geriatric patients, with an average caseload of 5-7 patients daily
- On-call: Rotation may be required, to be discussed during interview
- Territory: Up to a 60-mile radius from the designated hub or office; travelers expected to be flexible with territory
- Orientation: Mandatory Monday orientation and billable modules completed during orientation
- Submission: All submittals must meet Tier1 Travel Compliance requirements and be approved by HR
