Job Details
- Position Description: Home Health Physical Therapist responsible for patient assessments, treatment planning, and therapy implementation in a home care setting within West Portland, OR, and neighboring areas.
- Required Qualifications: Valid OR license, BCLS certification, minimum 1 year of skilled home health experience, proficiency in Homecare Homebase (HCHB), Oasis documentation experience, active drivers license, reliable transportation, and Monday orientation attendance.
- Schedule: 8-hour shifts, Days shift, minimum 40 hours per week.
- Additional Information: The role involves minimal driving, with an occasional maximum radius of roughly 60 miles per day. Candidates should be prepared for potential on-call rotations and RTO (Requested Time Off) to be submitted at application.
- Submission Requirements: Candidates must have their license in-hand at the time of application and complete the Monday orientation. This position is open to first-time travelers. Travel pairs are accepted but assigned to different territories with individual transportation.
